Patient Information

Please arrive for your first appointment 15 minutes early to register as a new patient. Upon arrival, check in at the registration desk in the main lobby and indicate that you are a new patient. You will then be asked to complete patient information forms including insurance and other information.

Payments for medical and surgical services are required at the time of service. WellGroup HealthPartners accepts several payment methods including: cash, check, Discover, MasterCard or Visa.
