At WellGroup, we are passionate about a healthy you and we work diligently to keep you well.  However, your health depends on much more than just the healthcare services we provide.  It requires your full participation in your care.  Remember, the choices you make today will affect your health status tomorrow.

As a patient of WellGroup HealthPartners, you have certain Rights and Responsibilities, highlighted below, that will help ensure the effectiveness of your care and your satisfaction with the course of treatment. 

You have the right to be treated with joy, respect, dignity, fairness and compassion. This means you should be greeted in a warm and friendly manner, and that physicians and others will use tact, sensitivity, sound judgment, and a professional attitude when interacting with you.

You have the right to be treated in a clean, comfortable and safe environment.

You have the right to receive full and complete information regarding you health status and any treatments or tests that may be performed. Do not hesitate to ask any question or voice any concerns during or after a visit.

You have the right to confidentiality. WellGroup HealthPartners complies with all applicable rules and regulations affecting your personal information.

You are responsible for providing complete and accurate information about your current and past illnesses, hospitalizations, medication and other matters related to your health status.

You are responsible for requesting additional information or clarification about your health status or treatment if you do not fully understand information or instructions.

You are responsible for following prescribed treatments and informing your physician and other caregivers if you anticipate or experience any problems in following those treatments.

You are responsible for providing necessary information for insurance claims and for working with the WellGroup to make payment arrangements when applicable.
